My Approach - The Formula? Less is More
Over the 25+ years that I've been a plastic surgeon, no procedure has changed more dramatically than blepharoplasty. In the past, many plastic surgeons removed too much tissue, which over time left patients with eyes that appeared sunken and hollowed.
The good news is that these problems can be avoided and often corrected. My formula is to remove less eyelid skin and fat and to preserve the eyelid muscle. The tissues around the eyes are lifted, restoring a youthful appearance.The endoscopic brow/midface lift (EBMF) works seamlessly with eyelid surgery to promote a youthful, rested appearance while providing support and redefining the lower lid minimizing the amount of surgery required on the eyelids.
Are You a Candidate?
It's not just in Dallas / Fort Worth that eyelid cosmetic surgery is popular. Nationwide,it is the most popular plastic surgery procedure for patients over 35. The skin around the eye is very thin and therefore more prone to show sun damage, lack of elasticity, and may even sag from pulling too hard on the skin when washing your face or applying makeup. Add to that the natural deterioration that comes with age and the eye area becomes the easiest area in which to see change. Men and women over 35 who are healthy both mentally and physically, and who are realistic in their expectations, are outstanding candidates for eyelid surgery. Patients under 35 are also good candidates if they have a family history of droopy, baggy eyelids.
So, how do you know if you are a candidate for blepharoplasty? Look at pictures of yourself taken 10 years previously and note the presence or absence of fullness in your lids. Look for loose or crepy skin that was not there before. Note also if the brows have descended to a lower level. If you are seeing any of these changes, a consultation is warranted.
How the Procedure is Performed
The Consultation
During your initial consultation, an examination of the relationship between the brows and upper eyelids and between the midface and lower eyelids will determine the plan of action. If the eyebrows are at the appropriate level, but there is excess skin or fat in the upper lid, then an upper lid blepharoplasty is the procedure of choice. It the lower lids have pockets of fat and loose or excess skin which allows the fat to bulge out, then lower eyelid surgery to remove or redistribute the fat is indicated.
The Day of Surgery
Prior to sedation, marks will be made around your eyes to indicate the incision sites following the natural lines and creases of the eyelids.
Surgery
Eyelid surgery carries minimal risk and can be done as an out-patient procedure in any accredited facility using IV sedation or general aesthesia. It is frequently combined with brow lift and face lift surgery in Dallas to achieve the proper balance in these areas. The majority of my cases are done at Dallas Day Surgery Center. The staff is exceptional and extremely skilled and the facility is very comfortable for our patients and their families.
- Upper Lid
To remove excess skin in the upper lid, an incision is placed in the natural fold where the overhang of skin is marked. The result is a fine, practically invisible scar hidden in the lid fold. - Lower Lid
The lower lid may be managed in two ways. If there is loose skin in addition to fat bulges, an incision is made just below the eyelid which allows the removal of skin. If there are fat bulges, the fat can be removed from a small incision inside the eyelid. This approach preserves lower lid support and is very effective at eliminating the appearance of fatigue. Incisions are closed with fine sutures leaving nearly invisible scars.
