This 31-year-old Dallas wife and student came to her initial consultation expecting to discuss a simple cosmetic rhinoplasty. Although she reported that she had some nasal trauma as a child, she had suffered no breathing or airway issues. She just wanted her nose straightened. It all sounds so simple. Upon evaluating her nose, a deviated septum was discovered requiring a septo-rhinoplasty procedure to correct. Septal cartilage grafts were necessary to address the internal damage. As the consultation discussion continued it was discovered that the aesthetic issue involved more than just straightening the nose. The dorsal hump would need to be reduced, the bulbous tip refined and the nostril size adjusted to arrive at an aesthetic look that the patient and the surgeon were pleased with. Some patients do require only a "simple cosmetic rhinoplasty", but it's important that both the surgeon and the patient keep an open mind and to go where the consultation leads them. Correcting the cosmetic portions without addressing the internal nasal structures would be wasting the rhinoplasty procedure. It's much easier to do the correct procedure the first time instead of having to do a secondary or revision rhinoplasty.
